Case Managers

To start receiving home-delivered fee meals from Texas Choice Solutions, Inc., please provide your case manager with the following provider information:

If you are already receiving meals from another provider and you want to switch to our company ask your case manager to submit an authorization on your behalf to us so that we may start meals for you.

Case Manger Section Only When submitting paperwork, please include the following:

Client address, phone number, date of birth
Agency and case manager information
Beginning and end dates of service
Number of meals client is approved for per week
Menu information or any special requests
Other critical billing or delivery information

IMPORTANT:Clients will receive a call from a CSI Meals team member welcoming them to the program.
